Target Audience:
This program is intended for pharmacists and other health care professionals who have little or no experience in geriatric pharmacy or have had an extended absence from geriatric pharmacy.
Learning Objectives:
At the conclusion of this application-based activity, participants should be able to:
- Discuss the range of socioeconomic status in today's older adult population and how it impacts health outcomes in older adults.
- Select and use appropriate screening tools to identify socioeconomic barriers to care in older adults.
- Identify opportunities to minimize disparities in care based on patient socioeconomic status.
- Provide input to other members of the interdisciplinary team based on assessment of socioeconomic barriers to care.
- Develop a care plan that considers a patient’s socioeconomic status and incorporates appropriate strategies for addressing barriers to optimal care.
