Target Audience:
This program is intended for pharmacists and other health care professionals who have little or no experience in geriatric pharmacy or have had an extended absence from geriatric pharmacy.
Learning Objectives:
At the conclusion of this application-based activity, participants should be able to:
- Develop and apply systems for the following: Medication reconciliation during transitions of care; Identification of risk factors for Adverse Drug Event (ADE) or medication incidents/errors; and Prevention of ADE or medication incidents/errors.
- Recognize iatrogenic conditions (e.g., healthcare associated infections, falls, pressure ulcers, medication-induced conditions).
- Develop strategies to prevent or resolve iatrogenic conditions.

CPE Post-Test: You will have two attempts to achieve a 70% or higher. You will not receive answer feedback until you attain this score.
CPE Credit: You must complete the post-test and evaluation within 365 days post purchase or its expiration date of May 1st, 2023, whichever occurs first to receive credit.
