The 4Ms is an evidence-based approach to care planning for older adults that's part of the Age-Friendly Health Systems initiative. Patient satisfaction improves and there is lower inpatient use and cost. National organizations will share their experiences with the 4Ms movement to advance pharmacist-provided 4Ms care.

Target Audience
Senior Care Pharmacists who provide expert advice on the use of medications to individuals and older adults and are committed to providing person-centered care, considering the complex interrelationships between disease states, nutrition, medications, and other variables.

CPE Credit:You must complete the post-test and evaluation by the expiration date to receive credit. You have three attempts to achieve a 75% or higher on the post-test. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. It is the responsibility of the participant to provide their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.
