Pharmacists Defying Gravity: Fall Assessment and Prevention in Older Adults
A fall is a common occurrence for older adults and can carry significant risk of injury or even death. Medications can become a risk factor contributing to a fall, thereby calling on the pharmacist to be able to assess fall risk in patients and to find and resolve any medication-related contributions that may increase risk. Through case studies, this course is focused on providing the participant in any practice setting with information useful to enhance the pharmacist’s skills in assessing fall risk in older adults, and in identifying medications which may add to a patient’s overall risk of falling.
Learning Objectives
At the conclusion of this application-based activity, participants should be able to:
- Discuss the role of comprehensive falls risk assessment and intervention in their practice utilizing falls prevention toolkits from the Centers for Disease Control and Prevention’s “Stopping Elderly Accidents, Deaths, and Injuries (STEADI)” initiative.
- Identify common risk factors for falls in the senior population.
- Identify medication related risk factors for falls.
- Utilizing patient cases, develop individualized recommendations for fall prevention based on a medication review.
