Learning Objectives
At the conclusion of this application-based activity, participants should be able to:
- Define the core social determinants of health (SDOH): socioeconomic status, physical environment, health and healthcare, social and community context, and education.
- Discuss the importance of addressing SDOH in order to support social and physical environments that promote better health outcomes.
- Describe how pharmacists can address SDOH inequities at the patient, practice, and community levels to reduce health disparities when conducting Transition of Care visits.
- Discuss the advantages of in-person pharmacist-patient interactions over telehealth visits when addressing SDOH.
Target Audience
Senior Care Pharmacists who provide expert advice on the use of medications to individuals and older adults and are committed to providing person-centered care, considering the complex interrelationships between disease states, nutrition, medications, and other variables.
