A Multifaceted Approach: The Pharmacist’s Perspective in Addressing the Social Determinants of Health

At the conclusion of this application-based activity, participants should be able to:

• Define the core social determinants of health (SDOH): socioeconomic status, physical environment, health and healthcare, social and community context, and education.
• Discuss the importance of addressing SDOH in order to support social and physical environments that promote better health outcomes.
• Describe how pharmacists can address SDOH inequities at the patient, practice, and community levels to reduce health disparities when conducting Transition of Care visits.
• Discuss the advantages of in-person pharmacist-patient interactions over telehealth visits when addressing SDOH.
