Learning Objectives:
At the conclusion of this application-based
activity, the participant will be able to:
- Evaluate findings from a comprehensive history and physical exam.
- Assess cognition, mood, physical function, nutrition, and pain utilizing valid and reliable tools/instruments.
- Recommend and interpret laboratory tests in the older adult, as well as identify those situations where patient-related factors should override standard recommendations for screening/treatment.
- Compose and review a complete medication history, paying attention to potentially inappropriate medications (PIMs) and other medications which can contribute to geriatric syndromes and conditions.
- Perform the role of the consultant pharmacist during transitions of care, including identifying potential hazards of hospitalization for older adults.
