Target Audience:
Pharmacists
with practices and businesses caring for older adults in hospice, assisted
living, skilled nursing facilities and home care venues of long-term care will
benefit from this presentation. Understanding various clinical quality barriers
and how to overcome them is complex given thee diverse population, resource
limitations and multiple payer scenarios in LTC.
Learning Objectives:
At the conclusion of this Application-Based activity, participants should
be able to:
- Formulate strategies to overcome clinical quality barriers in LTC.
- Evaluate the relevance of clinical practice guidelines, standards of care, and quality measures to geriatric care.
- Distinguish key characteristics important to collaborative practice in LTC.
- Integrate ethnic, racial, and cultural factors into health care decisions for older adults.
- Analyze why current LTC systems of care have associated risks of adverse drug events and medication errors and how they can be prevented.

How to
Earn Credit:
BCGP Credit: Per the Board of Pharmacy Specialties (BPS), all
pharmacists seeking BCGP Recertification have ONE attempt to achieve a 70% or
higher on the 10-question post-test.
CPE Credit: You must complete the post-test and evaluation by February 1, 2021 at 11:59 pm ET to receive credit. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. It is the responsibility of the participant to provide their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.
