Target Audience:
Senior
care and board-certified geriatric pharmacists who work with older adults and
health care providers to improve drug therapy. Focus is on community-based
practitioners, but tools can be applied to any setting.
Learning Objectives:
At the conclusion of this application-based activity, participants should
be able to:
- Review a variety of resources available to reduce medication-related problems in the older adult.
- Compare and contrast the strengths and weaknesses of evaluation criteria.
- Recommend ways to optimize drug therapy to an interprofessional team.
- Apply the explicit criteria to comprehensive medication reviews through patient cases.
The American Society of Consultant Pharmacists (ASCP) is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education.

How to
Earn Credit:
Post-Test: You will have unlimited attempts to achieve a 70% or higher
on the post-test assessment.
CPE Credit: You must complete the post-test and evaluation within 365 days post purchase or by its expiration date August 21, 2022, whichever occurs first to receive credit. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. It is the responsibility of the participant to provide their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.
