Target Audience:
Senior care and board certified geriatric pharmacists who work with older adults and health care providers to improve drug therapy. Focus is on community-based practitioners, but tools can be applied to any setting.

Objectives:
At the conclusion of this Application-Based activity, participants should be

- Review a variety of resources available to reduce medication-related problems in the older adult.
- Compare and contrast the strengths and weaknesses of evaluation criteria.
- Recommend ways to optimize drug therapy to an interprofessional team.

Earn Credit:
BCGP Credit: Per the Board of Pharmacy Specialties (BPS), all
pharmacists seeking BCGP Recertification have ONE attempt to achieve a 70% or
higher on the 10-question post-test.
CPE Credit: PLEASE NOTE THAT YOU MUST COMPLETE BOTH THE POST TEST AND THE EVALUATION ASSESSMENT BY October 5, 2019 AT 11:59 PM ET TO EARN CREDIT FOR THIS WEBINAR.
ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. It is the responsibility of the participant to provide their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.
