Target Audience: ASCP is proud to release the Medication Safety during Transitions of Care (MSTOC) Toolkit. This activity will orient members on using the Society’s online Practice Resource Center to access this collection of resources on clinical, billing and technology considerations for transitions of care. The MSTOC toolkit is targeted toward senior care pharmacists currently practicing or interested in transitions of care.
Learning Objectives:
At the conclusion of this knowledge-based activity, participants should be able to:
- Describe the transitions of care landscape
- Explain the importance of medication safety processes during transitions of care
- Review the components of ASCP's Medication Safety during Transitions of Care (MSTOC) Toolkit

How to Earn Credit:
CPE Credit: You must complete the evaluation by December 10, 2017 at 11:59 pm ET to receive credit. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. It is the responsibility of the participant to provide their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.
